My Design modifications were that I created 2 full new levels.  The first one acts as a tutorial, using the score pickups to guide the player.  The second is more exploratory, requiring the player to collect 3 keys to complete it, which are scattered throughout a world.

My aesthetic/gameplay modification was a new pickup.  This was a bad med kit, and it takes away a heart.  I use it in two different ways.  The first way is as an obstacle to avoid, or a pickup that you don't want to collect.  The second way is a pile of them to be used as a reset.  By collecting multiple your character will die, and go to the last checkpoint.  I had to create a new sprite for it, and write a new script that took away health instead of adding it.
